Easy IV cannulation Step-wise

IV Cannulation is a simple skill yet it needs thorough knowledge and practice to perform it in the right way. It is so important that it could be lifesaving in emergencies. Here I will share an easy IV Cannulation step-wise in really simple words. You’ll just have a mannequin or a real hand to assess your skill. But before trying IV Cannulation on a human Hand I would strongly recommend practicing it on a dummy or mannequin a couple of times until you are sure about your skill. The best is to perform it under the supervision of an expert.

The material you’ll need:

  1. An IV cannula
  2. alcohol swab
  3. pair of sterile gloves
  4. cannula fixing tape
  5. tourniquet

Easy IV cannulation Step-wise:

Step 1 (wear Gloves)

Doctor-patients safety always comes first. Wear a sterile pair of gloves to prevent cross infections and carry on the procedure under clean and sterile conditions.

Step 2 (choose the site)

Selecting the site where you are going to put the cannula is important. While deciding on the site for IV cannulations, you must take care of the following things.

  1. site of the Cannula should not be near the joint, because bending the joint can kink the cannula within the vein and cause obstruction.
  2. the vein should be straight and not branched
  3. there should be no valve in the way of the cannula

commonly used veins are basilic and cephalic veins on the dorsum of your hand, however, if due to any cause those veins are not accessible, alternative veins can be used in that cases.

Step 3 (apply the tourniquet)

Once you have selected the right vein, apply the tourniquet proximal to the selected vein. Ask the subject to clench the fist a couple of times or tap the vein gently a few times. This will cause pooling of blood in the distal veins, making them prominent and dilated. This will facilitate easy cannulation.

Step 4 (clean the site)

Clean the area around the vein with an alcohol swab in a circular fashion to kill the germs so that the bacteria from the skin are not taken into the vein along the cannula.

Step 5 (insert the cannula)

Now comes the most tricky part. For the insertion of the Cannula have to take out the cannula from the cover. Hold the cannula from the wings but make sure you don’t touch the needle. The bevel of the needle should face upward. Insert the cannula in the direction of the vein making a 15-degree angle with the skin surface.

Insert the cannula until you see blood in the flashback chamber. Stop at this point when you see blood.

Slightly retract the stylet and then proceed with the cannula inside the vein.

Step 7 (remove tourniquet and stylet )

Once the cannula is completely inserted, remove the tourniquet, and apply pressure on the vein (with the thumb of the other hand or ask someone to assist you) where the cannula’s catheter is placed. Gently withdraw the stylet.

Blood may still start to come out even though you have applied pressure on the vein with the thumb of the other hand but you don’t have to panic. It’s alright, just place the lock plug at the site from where you have removed the stylet.

Step 8 (flush the cannula)

Now we check if the cannula is in the right place or not, that is if the cannula is inside the vein or it is in the skin or subcutaneous tissue.

Take a 10 cc syringe, and fill it with normal saline. inject it in the cannula. If you don’t see any swelling, Voila, you did it the right way. But, if you see the tissue around the cannula swelling it means the cannula is not in the vein, it is in the wrong place. You have to take out and perform the IV cannulation again but at a different site.

Step 9 (secure the cannula)

After confirming that the cannula is in the right place by flushing you can now secure it with a cannula fixing tape.

Your IV cannulation has been successfully done now.

It is better to put the cannula in place rather than needle pricking the patient several time to administer the medication. Cannula is more convenient and less painful rather painless, and you can administer the medicine several time .
